Forms

In order to fulfill UCI and NIH regulations, all volunteers of the Laboratory for Fluorescence Dynamics (LFD) are required to complete and/or sign the following forms, and return them to the user coordinator respectively submit them online.

• UCI Waiver of Liability, Assumption of Risk and Indemnity Agreement 15 KB
• Acknowledgement Statement 8 KB, for having received and read the UCI Health System's Code of Conduct 126 KB
• Acceptable Computer & Network Use Policy 31 KB

Safety

Volunteers are asked to read the following safety sheets provided by the UCI Environmental Health and Safety Division at http://www.ehs.uci.edu/.
