Service Guidelines and Policies
Forms
In order to fulfill UCI and NIH regulations, all interns and volunteers working at the Laboratory for Fluorescence Dynamics (LFD) are required to complete and/or sign the following forms, and return them to the user coordinator respectively submit them online.
- HSSoE Volunteer Application and Emergency Contact & Medical Information
- HSSoE Waiver of Liability
- EH&S Visitor Authorization Form
- Acknowledgement Statement
, for having received and read the UCI Health System's Code of Conduct
- Acceptable Computer & Network Use Policy
The Online Service Form at https://www.lfd.uci.edu/service/form/ must be submitted for each independent project.
Safety
UCI employees are required to have current certifications from the UCI Learning Center (UCLC).
Non-UCI interns and volunteers must read the following safety sheets and complete online training modules provided by the UCLC. The user coordinator will request Student and Affiliate Access through the UCLC.

Insurance
All interns and volunteers must provide proof of medical insurance coverage. For health insurance coverage refer to:

Publications
In accordance to the NIH Public Access Policy, all final peer-reviewed journal manuscripts that arise from work at the LFD must be submitted to the digital archive PubMed Central upon acceptance for publication.
